We are excited to offer an innovative opportunity to enhance primary care delivery to care homes in the borough of Ealing. As a Care Home GP, you will value having dedicated time in your allocated care homes to build relationships with residents and staff and provide consistent, high-quality care. This is far more enjoyable than the pressures of ad-hoc care home visits which you may have experienced in other roles! Trust us on this, we have experienced that too.

As a valued member of our multidisciplinary team, you will receive robust support from fellow GPs and an experienced group of nurses, including advanced nurse practitioners, prescribing pharmacists, and administrators.

Who we are looking for:

· A newly qualified GMC registered GP.

· A diligent and reliable team member.

· A commitment to patient-centred care.

· An interest in frailty or medicine of the elderly (although this can be developed during the post)

· Face to face routine weekly consultations for patients identified as needing a clinical review in the care homes.

· Telephone consultations both for acute triage and planned care.

· Checking and signing repeat prescriptions electronically

· Prescribe generically whenever appropriately and in accordance with the Practice prescribing formulary, NICE guidance and Good Prescribing Practice.

· Dealing with queries, paperwork and correspondence as required including clinical administration and private report writing when agreed.

· Make professional, autonomous decisions in relation to presenting problems.

· Screen patients for disease risk factors and early signs of illness

· Clinical management of the patients in a care home setting as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate

· Develop care and treatment plans in consultation with patients, care homes and relatives in line with current clinical guidelines and protocols including end of life care and utilising the UCP platform.

· Participate in clinical audits, and learning event analyses and practice meetings

· Work with and provide support for other members of the primary health care team

· Play a lead role in the practice achieving agreed performance indicator targets (QOF, DES, Enhanced Services)
