1.Clinical Leadership:Provide clinical leadership within the practice, ensuring high-quality patient care and adherence to clinical guidelinessupported by the delivery of several clinical meetings. Activity:In collaboration andsupportedbyclinicalpartnersand administrative teams,help Chair practice and PCN MDT meetings; Practice Clinical Meeting (frequency dependent on site),practice Complex Care ICM MDT (monthly), practice palliative care MDT (quarterly), practice Safeguarding MDT (quarterly), PCN Proactive Care MDT (monthly) and PCN High Intensity User MDT (monthly). Ensure consistency of minute taking of meetings,sign off meeting minutesandmaintainaction log where appropriate.Help ensure dissemination of best practice (egNICE updates,drug safety alerts, complaints, deaths, new cancers, safeguardingcasesandNHSE SOPs)as standing agenda itemsthrough schedule of clinicalmeetings. Meet with other clinical leads across sites toenable peer support and ensure consistency of best practice across sites. Report through theSite-specific Partner into the Medical Director andExecutive Board. 2.Clinical Governance:Oversee clinical governance activities, including monitoring and improving clinical standards, patient safety, and the quality of care provided. Provide support and leadership with CQC inspections. Activity:Being the namedSafeguardingLead of thePractice, ensuredeliveryand adherence with organisationwideSafeguardingpolicy reporting into Organisational Safeguarding Lead as required for escalations. Working alongside SafeguardingAdministrativelead,ensuretimelycorrespondencewith externalstakeholdersand disseminationoflearningwith widerclinicalteams.SupportSenior Management Teamand Lead Pharmacistin reportinginto Executive Boardon key clinical KPIs and performance activity (e.g. delivery of QOF as well as local LESs). Support Lead Pharmacist with undertakingof 2cyclic audits to support CQC compliance requirements. Undertakequarterlyrecord keepingaudits based on NHSE guidance of GPs and LocumGPsand feedback areas of concern or improvement to Sie Partner.3.Team Management:Manage and support clinical staff,especiallyGPsbut also other Allied Health Professionals and learnersto ensure efficient and effective practice operations. Activity: Act as a key point of contact forclinicianswithin the practicetoescalateconcerns orqueriesaboutclinicalprocesses,SOPsor protocols.Supportinunblockingany barriersinefficientclinicalservicedelivery andescalatetosite Clinical Partnerif furthersupportor resource needed. 4.Clinical Decision-Making:Participatein clinical decision-making, including complex patient cases, referrals, and treatment plans. Activity:Undertake regularpatient facingclinicalsessions aswell as Duty Triage sessionsto ensure an authentic understanding of service delivery within the practice.Providesupportandsupervisionin dailyclinicalhuddlesas well asdirectlysupportingclinicalmeetingsanddisseminationofbest practice,clinicalprotocolsandSOPs to the widermultidisciplinaryclinicalteamincluding learners. 5.Clinical Audit:Lead orparticipatein clinical audits to assess and improve the quality of care provided within the practice. Activity:Supportoverseeinggeneralauditprogramwithinthe practiceboth for CQCcompliance(2cyclicaudits) anddelegationof otherrelevantauditsbased onpracticeneedsto thewider teamincludinglearners. 6.Education and Training: Provide guidance, training, and professional development opportunities for clinical staff to keep them updated with best practices and guidelines. Activity:Supporteducationandtrainingforleaners within thepracticeandcontributetomaintaininghighqualityeducationalstandardsthrough huddles andclinicalmeetings.Encourage attendance and engagement in local PLT events andfeedback any key areas of best practice or updates to local service delivery pathwaysthroughmeetings and via MS Teams. 7.Service Development:Contribute to the development and implementation of new services and initiatives within the practice to meet patient needs Activity:Supportthe practice with implementationofnew clinical service delivery modelsincollaborationwith the patients if necessaryand helpidentifygapsin service provisionthat maywarrantserviceredesignsupported througha QImethodologyapproach. 8.Quality Improvement:Identifyareas for quality improvement, implement changes, andmonitoroutcomes to enhance patient care. Activity:Supportthe seniormanagementteam todeliverpractice-basedQIprogramsthatrequireclinicaloversightand support writing up and sharing initiativeswiththewider team / organisationand system partners. 9.Patient Engagement:Collaborate with patients and gather feedback to ensure that clinical services are patient-centred and responsive to their needs. Activity:Supportpartnership and engagement with PPG through Administrative lead and support from Clinical Partners to deliver change ideas.Supportpractice-basedservice redesign based on GPPS, NHS choices, Googlereviews, FFTand other patient feedback modalities. 10.Collaboration:Work with PCN, Federation and other providersrepresentingthe practice tofacilitateintegrated care and promote better health outcomes for patients. Activity:Work alongside otherclinicalleads within theorganisation to ensure consistency of care and service delivery in relation to PCN based clinical contracts supported by SMT, PCN manager and Clinical Partners.
