Job Summary: The post holder will deliver evidenced based care to the practice population in conjunction with other members of the Practice team. They will be expected to work autonomously abiding by the NMC Code of Conduct, using Practice guidelines and Protocols. Professional Responsibilities Support and implement evidence-based guidelines in conjunction with the Lead Nurse and Practice Nurse Team. Ensure personal development using evidence-based guidelines, study days, reading and other appropriate methods of learning and education. Support the HCA / SNA / NA team members in their professional development and learning. Recognise opportunities to enable partnership working with all members of the Practice team. Participate in annual appraisal process. Be able to identify your own training needs in line with Lead Nurse and partake in the training identified. Take responsibility for your own personal training needs and help to identify that of others less senior to your role. This includes inhouse Blue Stream training which is mandatory. Maintain professional development by attending training appropriate to the role including University modules, study days, webinar and reading of appropriate literature. Actively participate in multidisciplinary education and training Maintain a professional development portfolio and undertake revalidation with the Nursing and Midwifery Council every three years. Leadership Provide leadership and mentorship to HCA / SNA / NA team, including treatment planning of their patients on request, ensuring all advice and guidance given is within the scope of their practice and underpinned by evidenced based practice. Mentorship of newly qualified staff and student nurses. Clinical All care administered to a patient must only be carried out following informed consent. Gain a basic understanding of the Quality and Outcome Framework to enable support to the Practice to achieve maximum points and provide patients with the care underpinned by this framework. Undertake opportunistic screening where clinically indicated, with informed patient consent. Wound management including chronic and acute wounds, removal of sutures and staples. Leg ulcer management, compression bandaging and measuring for hosiery. Able to undertake doppler assessment. A knowledge of contraception for all stages of life. Undertaking oral contraceptive checks and administration of depo injections. A knowledge of Sexual Health including how to obtain specimens for routine STI screening and the knowledge of how to refer patient for contact tracing if appropriate. The ability to recognise when a patient should be referred to secondary care services. Cervical screening. Administration of both adult and paediatric vaccinations using Patient Group Directive (PGD) or Patient Specific Directive (PSD). This will include Paediatric vaccination clinics for babies and pre-school children. Have a working knowledge of safe storage of vaccines, stock rotation and disposal of same. Administration of injections specific to individualised patients using PSDs, this would include such drugs as Vitamin B12, Nebido, Prostap, Zoladex. Provide lifestyle advice to enable patients to maintain optimum health, using outside agency resources as appropriate. Maintain complete and contemporaneous documentation of all patient contacts whether by phone or face to face. The Practice Infection, Prevention and Control protocols must be adhered to throughout all aspects of this role. Safeguarding the postholder must always be up to date in Safeguarding Level 3 training. Other: venepuncture, ECGs, urinalysis, processing of laboratory specimens. The postholder may be asked to undertake home visits for patients that are unable to attend the surgery and do not meet the remit or skills of the district nursing services. Anaphylaxis and Resuscitation training must be undertaken annually to enable the postholder to remain in this role. All the above are dependant on adequate training and assessment being met or proven. The postholder must be both competent and confident to undertake any task they attempt. If there is any doubt speak to either the Lead Nurse or Clinical lead. Research, Audit and Quality Be willing to take part in audit within the Practice to improve patient care and outcomes. Contribute to the provision of equitable, evidence-based services for all patients within the Practice. Ensure all care is evidenced based using contemporaneous literature or education. General Ensure all working areas are kept clean and tidy. This will include appropriate stock levels and rotation in your working area. Maintain a complete knowledge of all protocols and procedures applicable to the Practice Nurse role. Comply with all clinical waste and infection control policies. Process referrals to appropriate external agencies to enable complete patient care. IT and Computer Literacy Maintain a working knowledge of System One computer system to enable complete care of the patient including correct documentation, use of READ and SNOMED codes to ensure Practice finances are maintained for services supplied. Have a working knowledge of phone system to enable the receiving, transferring, and making of calls. Be fully conversant in the use of emails. Full adherence to the Practice Information and Governance policies. NOTE: The practice operates several policies and procedures (see below). Breach of these may lead to disciplinary action, and in some cases will be gross misconduct, which may lead to dismissal. Health & Safety: The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety Policy, to include: Using personal security systems within the workplace according to practice guidelines. Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks. Making effective use of training to update knowledge and skills. Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards. Reporting potential risks identified. Communication: The post-holder should recognise the importance of effective communication within the team and will strive to: Communicate effectively with other team members. Communicate effectively with patients and carers. Recognise peoples needs for alternative methods of communication and respond accordingly. Contribution to the Implementation of Services: The post-holder will: Apply practice policies, standards, and guidance. Discuss with other members of the team how the policies, standards and guidelines will affect own work. Participate in audit where appropriate. To cover for colleagues during their absence due to leave or sickness when requested. This will entail a need to work outside of normal contracted hours.
