Crynodeb
Accountable for designated caseload. Undertake dietetic assessments. Assess and manage nutritional risk effectively in community settings. Advise and liaise with inpatient setting such as local acute trusts and specialist ED collaborative. Develops and implements individual and/or group dietetic interventions, in collaboration with the service user(s) and MDT to achieve therapeutic goals. Monitors and evaluates dietetic interventions modifying them according to need. Works within HCPC Code of Professional Conduct and Professional Standards, accountable for own professional practice including work delegated to others. Works within occupational policies, procedures and protocols, using discretion as appropriate. Participates in regular clinical supervision.
