Crynodeb
Provide senior triage nursing support to our team of multidisciplinary professionals providing clinical health and social care in admission prevention / avoidance service. Maintain standards of care and to deploy, co-ordinate and supervise the nursing team and learners as required. Be responsible for the assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community. Be highly experienced in management of a wide range of community pathways and provide specialist advice for patient care. They will provide advanced complex nursing assessments which may include on occasion interpreting blood results. The post holder will, assist in triage of complex patients within all the community pathways which may include H at H pathways, having clinician to clinician discussions to facilitate effective referral into the most appropriate service to meet the needs of the patient. The post holder will support patients virtually and when required face to face.
