Crynodeb
Coordinate patient flow and discharge planning for complex cases across Neurology, Rheumatology, Renal, and General Internal Medicine specialties. Utilise OPTICA to monitor patient status, update discharge readiness, and maintain accurate electronic records. Update and manage the Ward Board daily to ensure visibility of patient progress and discharge priorities for the multidisciplinary team. Act as the primary liaison between patients, families, clinical teams, and external agencies to facilitate safe and timely discharge. Identify and resolve barriers to discharge, escalating issues where necessary to maintain patient flow and meet hospital targets. Arrange referrals and community support services, ensuring continuity of care post-discharge. Participate in board rounds and multidisciplinary meetings, providing updates and supporting decision-making. Ensure compliance with hospital policies and national standards, contributing to quality improvement initiatives. Maintain accurate documentation and communicate effectively with all stakeholders to support patient-centred care.
