About the Team This is an exciting opportunity to join our Learning Disability, Autism and Hearing & Vision Adult Social Care service in Keynsham as a Senior Practitioner. You’ll play a vital role in supporting adults with a learning disability, autism and/or sensory impairment, helping them navigate complex needs and accessing the right support, as well as providing a leadership role within the service. You’ll be part of a warm, collaborative, and forward-thinking team, working alongside a wide network of professionals including Community Learning Disability Teams, AWP Mental Health Teams and more. It’s a chance to make a real difference in the lives of people and their families. About the Role As a Senior Practitioner, you’ll manage a diverse and complex caseload and work with a high degree of autonomy, supported by experienced Team Managers and a Service Lead. This is a role where your expertise and initiative will be valued - and where your impact will be visible. Your responsibilities will include: Leading adult care assessments and reviews under the Care Act 2014 Designing personalised care and support plans Conducting Mental Capacity Assessments and Best Interests Decisions Managing risk through thoughtful, person-centred strategies Manage a varied, complex and higher-risk caseload where service users have diverse and cumulative requirements Completing Section 117 Aftercare reviews and Continuing Health Care Checklists Provide support, guidance and mentorship to more junior colleagues Leading on Safeguarding enquiries About You We’re looking for someone who is passionate about empowering adults with diverse needs and committed to high-quality social work practice. You’ll need: A recognised Social Work qualification (Degree, Diploma, CQSW or equivalent) Additional qualification and experience in area of specialism (e.g. Best Interest Assessor, Practice Educator) or willingness to work towards this Registration with Social Work England Significant experience supporting individuals with autism, mental health conditions, learning disabilities, or sensory needs and physical disabilities Experience of managing a complex caseload Some experience of providing mentoring and support to less experienced practitioners.
