The States of Guernsey are looking for a Palliative Care Specialist Nurse to join the Community Health and Social Care Service.
Adult Community Services is a multidisciplinary team with Community Nurses, Social Workers, Specialist Teams, Social Care and support staff on the same site. As a Palliative Care Specialist Nurse, the post holder will work collaboratively with multi-Professional teams in Primary Care, Oncology and the community including Hospice and ‘site-specific’ teams in the Bailiwick of Guernsey to provide evidence-based palliative and supportive care for patients/clients and their families, prioritising people as individuals. The post involves acting as a resource for palliative and supportive care nursing provision, supporting clinical leadership, teaching and contributing to policy development.
