Core Responsibilities Responsibilities and duties may include (and may not be limited to): Provide clear, evidencebased advice to GPs, practice staff and patients on prescribing changes to enhance safety, quality and costeffectiveness. Support the achievement of the Local Prescribing Service Level Agreement (SLA), prescribingrelated Quality and Outcomes Framework (QOF) targets, and any other locally agreed prescribing schemes. Prepare evidencebased resources and information to support rational, costeffective prescribing across the PCN. Contribute to the review, development and implementation of repeat prescribing protocols and Standard Operating Procedures to improve efficiency and effectiveness. Take a proactive approach to the safe handling of prescriptions, including identifying risks and recommending improvements. Advise on the safe sourcing, storage and management of medicines as required. Participate in medication audits as requested by the practice, Staffordshire & Stoke-on-Trent ICB or PCN pharmacists. Liaise with the Staffordshire and StokeonTrent ICB Medicines Management Team regarding local prescribing policies, guidance and incentive schemes. Support the introduction and optimisation of new prescribingrelated processes within the practice. Support the PCN Clinical Pharmacist in delivering medication reviews, developing patient information materials and promoting medicines awareness. Respond professionally to prescription queries from patients and staff, escalating to the appropriate clinician in line with practice protocols. Assist clinical staff with medicines reconciliation in accordance with practice procedures, ensuring accurate and timely updates to patient records. Additional Responsibilities Receive messages and relay information accurately and appropriately. Carry out data entry, data allocation, collation and processing in line with practice procedures. Initiate contact with, and respond to, patients, colleagues and external healthcare professionals. Contribute to maintaining a positive, patientfocused culture across the PCN. Support a positive, collaborative and supportive working environment within the PCN. This information is given in confidence, and all staff are expected to respect patient privacy and handle such information appropriately. In carrying out the duties of this role, the post holder will have access to confidential information relating to patients, carers, practice staff and other healthcare professionals. The post holder may also access confidential information relating to the business operations of the organisation. All such information, regardless of source, must be treated as strictly confidential. Confidential information relating to patients, carers, colleagues, other healthcare workers or the practice may only be shared with authorised individuals and strictly in accordance with organisational policies on confidentiality and the protection of personal and sensitive data. Identifying risks associated with work activities and carrying them out in a way that effectively manages those risks. Reporting any identified risks or concerns promptly through the appropriate channels.
