Crynodeb
The role will involve you: Completing holistic assessments of patients needs following hospital discharge or to prevent admission. Referrals also come via single point of access from the neighbourhood team, Social Services and GPs. You will be completing assessments for patients for low level equipment, adaptations and manual handling needs. Initiate and co-ordinate individual rehabilitation programmes and support and educate assistants to deliver these effectively Undertaking occupational therapy assessments for a designated caseload, addressing occupational performance and skill deficits, enabling the client in areas of self-maintenance, productivity and leisure. Assessing clients: in relation to their equipment and specialist equipment needs / for minor adaptations eg rails, half steps, and accora riser-recliner chairs. Working with clients to identify Occupational Therapy goals as part of the overall care plan. Monitoring and evaluating treatment in order to measure progress and ensure effectiveness of the intervention. Act as key worker for patients on the intensive support team to co-ordinate their ongoing needs by liaising with the patient, GP, MDT, relatives and other agencies. This role will include organising individual client case reviews, communicating plans with the MDT, and being responsible for the ongoing actions in relation to the patient pathway. Maintaining contemporaneous clinical notes to the high professional standards of the College of Occupational Therapy and in accordance with Trust guidelines. Line management of band 5 and band 4 team members including training, supervisions and appraisals.
