Crynodeb
Be responsible for completing the initial subjective/objective assessments on triaged noncomplex patients and complete relevant documentation following assessments to demonstrate clinical reasoning. Support registered staff in the assessment and intervention of more complexpatients and be responsible for the continuing assessment following a baseline assessment performed by registered staff. Undertake more complex interventions under the direction of theregistered Therapist. Work closely and liaise with all members of the Multi-disciplinary Team ensuring a seamless pathway of care. Effectively develop and implement a prescribed therapy plan including mobility/functional interventions to enable people to be safely rehabilitated in their home environment or residential rehabilitation unit. Participate in seven day and / or extended hours working if required to do so. Deal sensitively with patients who have high levels of anxietyand aggression caused by pain, dementia or limited mobility in frequently distressing or emotional circumstances, providing care for distressed patients clients or clients with challenging behaviour. Able to prescribe equipment and walking aids to enable patients functional safety and mobility. Promote dignity and privacy to all patients. Ensure responsive and timely interventions to people in their own homes or needing support to return home or to a safe environment. Work in unpredictable community settings where working conditions may be unpleasant
