Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. To be professionally accountable for the assessment, planning, implementation, and evaluation of individual patients with complex tissue viability matters. To organise the caseload efficiently and effectively with regards to clinical priorities and use of time. Providing expert advice and guidance relating to tissue viability to the patients, carers and health care professionals including community, residential and nursing home staff working closely with secondary care and social services. Act as the patients advocate when appropriate and collaborate with other health and social agencies to facilitate the best outcome for the patient. Promote awareness of the diverse patients with different needs and expectations and help the clinical manager to develop strategies to meet the challenges for providing high quality care. Ensure accurate and timely clinical records are maintained. Work with clinical manager to design and review evidence-based Tissue Viability policies and procedures that assure quality and meet professional requirements.
