Professional Workin accordance withNMC Code of Professional Practice and Scope of Professional Practice. Professionally and legally accountable and responsible for all activities. Work within the community (e.g. within clients own home) as a lone practitioner managing situations of risk with telephone support. Seek support and advice with cases from more experienced colleagues when faced with matters outside sphere of responsibility,knowledgeor skills. To provide a calm and supportive approach to the management and delivery of sensitive and complex information, providing both practical and emotional support, utilisingflexibilityand empathy indifficult situations. Use effective communication techniques communicate to the multidisciplinary team to ensure the child and family referred to Childrens Rapid Response receive the best possible care. Undertake risk assessments prior to service delivery within the home setting to ensure staff safety. Work in partnership with children,familiesand carers to provideoptimumindividual outcome acknowledging children and familys personal preferences, spiritual and religious beliefs. Formulate individualised management and treatment plans using sound knowledge andevidence basedpractice. Plans should be in response to sound clinical reasoning and include expected signs of deterioration. To providetimely, effective symptom management, resulting from a comprehensive history taking utilising independentnon medicalprescribing,GPsor hospital team, completing the symptom flow charts tomonitorthe childs changing condition/deterioration. Provide and promote strategies/detailed plans of care to parents and carers empowering care that will prevent inappropriate admissions and reduce length of hospital stay. Ensure assessment, planning and delivery of holistic care is of the highest standard utilising an agreed nursing model. Liaise and work in collaboration with acute services, primary healthcare team and other agencies when planning and implementing programmes of carefacilitatingearly discharge and preventing admissions. Advise the Team Leader, Clinical Lead or Service Manager of any circumstances which give cause for concern in relation to patient care and/or any concerns raised by the primary healthcare team or partner agencies. Maintain clear,conciseand contemporaneous records adhering to BCHC record keeping policies. Provide a flexible service to accommodate the changing needs of children and families. Management In the absence of the Team Leader/Clinical Lead provide leadership to ensure the effective service delivery. Responsible for accepting referrals and discharging patients from own caseload. Responsible for reassessing children, ensuring regular formal review of the caseload in conjunction with the Team Leader, ensuring these are documented andevidencedas per record keeping guidelines. Ensure standards aremaintainedwith both clinical quality and governance being central to the function and delivery of the service. Accountable for advice and guidance to patients, families,carersand other team members. Act as an effective role model for other team members,studentsand professionals. Participate in research and audit relevant to the service in conjunction with the Team Leader/ Clinical Lead. Appropriately manage the service resources e.g. equipment, staffing, supplies in conjunction with the Team Leader/Service Manager. Develop policies,proceduresand guidelines in conjunction with the wider multidisciplinary team. Ensuring staffmaintainorganisational standards in relation to record keeping. Undertake risk assessment for own clinical caseload, action planning effectively in response to risk assessment. Ensure effective communication to line manager relating any circumstances which give rise for concern. Manage informal complaints from service users raising any recurrent themes or concerns with the Clinical Lead and Service Manager andassistwith formal investigations asrequested. Demonstrate critical thinking, problem solving anddecision makingskills. Work positively with colleagues tomaintaineffective working relationships. Accurately record andmaintainclient records daily using electronic and paper systemse.gRiO. Education and Development Tomaintainand update knowledge and skills in relation to the post. Toidentifyand undertake specific training tomaintainskills in line with NMC Scope of Practice and development needs of the service. Participate in the teaching of students and other staff within the clinical and university environment. Participate in the teaching and mentorship of staff asrequired, ensuring they have the knowledge and skills to fulfil their roles. Maintain own professional development in line with post-registration ongoing education and practice, Revalidation. Act as a link Nurse and take clinical lead in areas of expertise. Attend courses and meetings asrequiredto fulfil the link role as delegated by the Team Leader. Participate in reflective practice/clinical supervision, critical incidentanalysisand debriefings. Management and Leadership Responsibilities Develop and empower all members of your team to perform tohigh standardsand innovate. Ensure supportive staff management arrangements are in place and carry out personal development reviews for direct reports. Ensure all staff in your team/s have annual PDRs resulting in specificobjectivesand effective personal development plans in line with the Knowledge and Skills Outline Framework. Develop staff knowledge and skills to promote equality and diversity and address inequalities both in employment and service delivery. Ensure specific equalityobjectivesare included inPDRs.
