Crynodeb
Working with GPs and other professionals within the practice to identify and manage a caseload of patients, particularly vulnerable patient groups with long-term conditions that may require extra support. Directing patients to locally available support and services. Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred. Assisting the Nursing Team with patient wound assessment and care Phlebotomy Vaccinations and injections where qualified eg Influenza & B12 Chaperoning duties Promoting health education. Maintain and clean equipment used by the nurses and GPs according to the practice Infection Control Policy. Surgical equipment re-stocking and stock rotation Infection Prevention Control / Undertake housekeeping duties, including general tidiness and cleanliness of treatment and GP rooms.
