Please see attached job description and personal specification for full details. The Neuro Navigator post holder will have a clinical case management role. They will support clients with neurological disabilities as they move from acute services through the whole pathway until they are settled in a permanent/long-term setting. The Neuro Navigator will: Act as a link person for acute and community colleagues, rehabilitation units and GPs and the continuing healthcare team understanding pathways accessed by all clients with a neurological diagnosis (both single neurological events such as stroke and clients with neurological conditions). They will have a role in liaising and networking with neurological services. Provide an expert assessment of neuro rehabilitation need, providing a solid understanding of the clinical impact of stroke and acquired neurological conditions. Work with partners where an assessment is required to ensure robust completion of appropriate assessments, clarity of changing need and appropriate management of expectation with the family regarding expected outcomes. Provide an expert neuro rehabilitation opinion on the appropriateness of rehabilitation referral supporting identification of an appropriate pathway, working in partnership with health and social care professionals to promote an integrated approach to management of progressive and non-progressive neurological conditions. Facilitate and support the referral process into the relevant rehabilitation pathway. Monitor and support rehabilitation admissions, including supporting the teams with discharge planning. To act as a specialised source of expertise on the management of service users within the specialist area of neuro-disability and provide a Neuro Navigation service to service users / carers, health, community care and voluntary sector workers to ensure the patient is accessing services in the right place at the right time. The post holder will have a core base within Hillingdon Borough but will be expected to visit hospital sites, community providers and patients homes within Northwest London and surrounding area. Where possible caseloads will be organised by geographical area. Communicate with service users, their families and carers appropriately empowering them to make informed choices about their health and healthcare, whilst actively promoting their health and well-being. Network with other health and social care providers, promoting an integrated approach to the management of complex neurological conditions. Provide education and training to health and social services working with this group of service users, to achieve more integrated cross organisational working. Have an overview of challenges in the system impacting flow and supporting clinical decision making. The post holder is a lone practitioner within this specialist field in Northwest London. They work autonomously within the standards of professional practice laid down by the relevant professional body and Healthcare Professional Council (or equivalent).
