Crynodeb
To independently manage a caseload of allocated service users with complex mental health needs and act as a care coordinator and Case Manager to service users To provide assessments of the service users environment, how their accommodation is meeting their needs and whether there are any unmet needs; presenting this to placement review meetings as required. To select and use validated standardised assessment tools routinely and in practice, exhibiting clinical reasoning skills; to include the Model of Human Occupation (MOHO) assessments. To plan and implement service user led interventions, using graded activity and a wide range of treatment techniques to achieve outcome focused recovery goals. To work with service users in a variety of settings in order to provide the most effective assessment and interventions To consider both physical and mental health needs as part of the OT assessment and to refer to appropriate agencies where there is need for equipment. To undertake assessment of daily living skills to ascertain level of functional ability, risk factors, level of support / accommodation needs to support the development of community care plans. To use clinical reasoning to assess how the clients mental health difficulties impact on their function in the occupational areas of self-care, productivity and leisure as well as their performance in the cognitive, inter/ intra personal and physical areas. To assess clients mental state and identify any significant risk factors pertaining to community integration and undertake risk assessments.
