Crynodeb

To be an integral part of the Community Integrated Neuro and Stroke Service. To provide evidenced based specialist therapy assessments assessment and appropriate therapy intervention to patients referred to the service. Follow defined stroke and neurological care pathways and other relevant rehabilitation pathways as directed by line management. To liaise closely with members of the interdisciplinary team and wider multidisciplinary team as required, facilitating effective caseload management. To utilise specialist skill and knowledge to manage a neurological caseload. To have general knowledge of a variety of conditions including neurological, orthopaedic, musculoskeletal, care of the elderly, respiratory and falls; understanding the impact of multi pathological presentation. To utilise a comprehensive range of evidenced based specialist assessments and treatment to patients in their own home or in a clinic environment. To assist in the quality and activity monitoring systems, to support evaluation of effectiveness of the service. To assist in the maintenance and development of the service. Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
