Main Tasks To be responsible for delivering high standards of care when accessing a Central Venous Access Device. To participate in formal education and teaching sessions to registered nurses on the care and maintenance of Central Venous Access Devices. To assess staff accessing Central Venous Access Devices and sign them off as competent. To maintain an exemplar level of communication with patient, family and the multidisciplinary team. To establish plans of care to the benefit of the patient and ward staff. Clinical Duties To undertake a comprehensive assessment of patients, participate in the assessing, planning, implementation, and evaluation of individualised programmes of care. To be responsible for delivering high standards of care when accessing a Central Venous Access Device. To support the senior team members on PICC insertion days. . Establish and maintain effective communication within the multi-disciplinary team. Act as an innovative and enthusiastic role model, providing leadership, guidance and advice to staff on clinical issues. Attend and participate in relevant meetings as required including providing feedback and disseminating information as appropriate To ensure that high standards of nursing care are given and maintained, and to act accordingly when standards are not being maintained. To recognise changes in a patients condition which require the intervention of others and refer on as appropriate. To act as an advocate for the patient and the family, including promoting and protecting their interests, rights, equality and inclusion and privacy Professional Duties To contribute to a culture that recognises and works in partnership with patients and their families, involving them in all decisions. To work within the NMC code of professional practice, and within policies, procedures and guidelines of Calderdale & Huddersfield NHS Foundation Trust. To be aware of and adhere to the Trust Policies relating to the control and administration of drugs. To ensure that high standards of nursing care are given and maintained, and, to act when standards are not being maintained. To participate in regular staff meetings. To be flexible to work within areas or departments (cross-site) to meet the demands of changing service needs. To take responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with re-registration requirements. To communicate with the team leader and line manager and collect data in order to participate in the smooth running and organisation of the service.
