Do you have the passion to make a difference to people’s lives following a neurological event?
The Community Integrated Neuro and Stroke Rehab Service (CINSS) are looking for a highly motivated Occupational Therapist to join their interdisciplinary team.
The service is a community based interdisciplinary therapy team, who support the timely discharge of patients from hospital. These patients have ongoing rehabilitation or neuro management needs following a new or worsening presentation of a neurological condition, both stroke, and non-stroke.
The teams work collaboratively with acute services, rehab and community networks and third sector services to deliver a patient centred; goal focused approach to rehabilitation. Our team is an integral part of the stroke pathway, in reaching into both stroke and in-patient rehab settings.
This post will offer an opportunity to consolidate and develop neurological and wider clinical skills, giving you the opportunity to be part of quality improvement projects and service developments, working closely with other Occupational Therapists and the wider team.
The suitable candidate may have some experience in managing a caseload of neurologically impaired adults and will need to be HCPC registered

Expectation to work independently and effectively within CINSS to ensure safe and effective management of patients in the community. You will be able to prioritise your workload and time effectively to support an integrated approach.

The successful candidate will have evidence of post-graduate experience and sound clinical reasoning with expectation of being committed to providing high standards of care.
The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. This pilot aims to explore team structures and assess the impact on patient experience. While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group.
If you are successful in your application, please be aware that your team structure and line management may be subject to change in the future as part of this evolving model. However, any formal changes will be preceded by appropriate consultation in line with organisational policy.

• To be an integral part of the Community Integrated Neuro and Stroke Service.
• To provide evidenced based specialist therapy assessments assessment and appropriate therapy intervention to patients referred to the service.
• Follow defined stroke and neurological care pathways and other relevant rehabilitation pathways as directed by line management.
• To liaise closely with members of the interdisciplinary team and wider multidisciplinary team as required, facilitating effective caseload management.
• To utilise specialist skill and knowledge to manage a neurological caseload.
• To have general knowledge of a variety of conditions including neurological, orthopaedic, musculoskeletal, care of the elderly, respiratory and falls; understanding the impact of multi pathological presentation.
• To utilise a comprehensive range of evidenced based specialist assessments and treatment to patients in their own home or in a clinic environment.
• To assist in the quality and activity monitoring systems, to support evaluation of effectiveness of the service.
• To assist in the maintenance and development of the service.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
