We work for Health and Justice services as part of Central and North West London NHS Foundation Trust (CNWL) and take great pride in the work that we do in a very challenging environment. If you think you would be able to work in a high-pressured setting, delivering all aspects of clinical care, then this could be the role for you.

We are looking forRegistered Adult Nurseswith senior experience at Band 5 to join us as part of the Primary Care team within the prison ofHMP High Down. We are able to offer supervision, training and preceptorship. You do not need to have prison experience as we will provide you with a full induction and training.

Please note that any candidate wishing to apply for a role within our team will have to complete prison vetting clearance. This requires that you have been a resident in the UK for a minimum of 3 years.

Primary care is responsible for screening all new arrivals, health promotion, long term condition management, minor ailments and responding to incidents wherever they may occur in the prison. Working alongside the Mental Health and Substance Misuse team affords us the benefit of being able to work in a truly holistic manner, responding to the needs of both physical and mental well-being in partnership. Our other partners include; GP, dentistry, optician, podiatry, physio and radiology. We also work closely with HMPPS staff, education and other professionals.

Our population is one which is either excluded from mainstream healthcare services or have very limited interaction with health services in the community. Your interactions could influence the way a man thinks about his future, thinks about his health needs, and feels about himself and others. You could make a real difference.

• Plan,deliverandevaluateclinicallyappropriateinterventionsusing evidenced basedguidance and practice.

• Set up processes/protocols for effective assessment,planning,implementationand evaluation of care for those patients with physical, mental health and substance misuseproblems.

• Safeguard vulnerable patients in line with CNWL Trust safeguarding policies.


• Apply local risk assessment and managementprotocols.

• Makeappropriate referralsto other services for long term or complex care management.

• Co-ordinatemulti-disciplinaryclinicalpracticetoprovidefocusedandintegratedcare treatmentprograms/packages.

• Use highly skilled interventions to de-escalate aggressive patientbehavioursand beawareoftheappropriateprocesses for dealing withthem.

• Work collaboratively with staff of otherdisciplinese.g.prison, educationstaffto provide integrated and coordinatedcare.

• Work withinownsphere of competency and ensure that training needs are communicated to the teammanager.

• Participate in local and trust audits and trust audits. Participate in clinical governance andutiliseresearch skills for audit, servicedevelopmentand research within the area served by the multi-disciplinaryteam.

• Liaise with other professionalservicese.g.hospital staff,GPsand caseworkers, sharing informationwhereappropriate.

• Work in a highly emotive environment with patients who may have a sexual offending history or mental health / substance misuse problems on a regular basis. Able to communicate sensitive and complex information whilstmaintaininga high levelof professionalism at all times.

• Attend and contribute to prison meetings asrequired, andpresent the view ofHealthcareTeam on relevant clinical issues.Ensureany actions taken from these meetingsarecommunicated totheappropriateindividualresponsible for completing the action.


• Ensure that pathways of care are created for the patients, including collecting collateral information and makingappropriate referralsinto the community when they are approachingdischarge.

• Work closely with colleagues to ensure information about risk is recorded and communicated sensitively andcomprehensively.

• Maintain the highest standards of clinical record keeping, including electronic dataentryandrecording.

• Ensure that within all aspects of professional activitye.g.communication with patients, relatives, colleagues and the assessment, development and delivery of interventions, issues of diversitysuchas race, culture, identity,genderand sexuality are sensitively considered and incorporated to reflect individual differences appropriately.


Research and Audits

• Utilisetheory, evidence-basedliteratureand research to support evidence-based practice in individualwork.

• Assistinaudits as per the annual planner or when requested to do soin order todevelop and improve service provision.


Teaching, Training and Supervision

• Participate in regular clinical and managerial supervisionin accordance withprofessional practice and CNWL guidelines.

• Attendappropriate businessand clinical meetings,supervisionand reflective practice groups


• Develop skills and competencies thatassistin the delivery of current duties

• Contribute to andfacilitatethe training and support of all staff.

• Manage and provide supervision and training to nursing staff.

• Ensure all appraisals are up todateand each member of the team has a personal development plan.

• Develop nursing skills and systems which allow for effective triage, minor alimentmanagementandappropriate referralto other healthcare services.

• Participate in the induction and training ofnew staff, peers, juniorstaffand students


Management, Recruitment and Service Development

• Assistin the design and implementation of service development projects within the service asrequired.

• Attendmeetings in which service developments are planned and discussed.


IT responsibilities


• Input andmaintainappropriate electronicclinical records and statistical data, providing this to the Service Lead/business and performance team when necessary.


Policy and Procedures

• Duties and responsibilities of this post will be undertakenin accordance withthe policies and procedures of CNWL / Health & Justice Service Directorate and withthe NMCCode of Conduct and Guidance on Professional Practice. It is the post-holder’s responsibility to ensure that they keepup-to-datewith these policies and other policydocuments.

• Worksin accordance withHealth & Justice Service Directorate procedures, in particular security,healthandsafety.
