We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in organising and managing care plans, ensuring seamless communication between clients, families, and healthcare professionals. This position offers an opportunity to make a meaningful difference in people's lives by providing exceptional support and coordination. Prior office experience is desirable to facilitate efficient administrative and organisational tasks.
Duties
• Coordinate and oversee individual care plans, ensuring all aspects of client needs are met effectively
• Act as a primary point of contact for clients, families, and healthcare providers, maintaining clear and compassionate communication
• Schedule appointments, follow-ups, and service deliveries in accordance with care requirements
• Maintain accurate records of client information, care activities, and progress notes using organisational software
• Assist with administrative tasks such as filing, data entry, and managing correspondence
• Collaborate with multidisciplinary teams to adapt care plans as needed and ensure continuity of care
• Monitor service delivery to uphold high standards of quality and compliance with relevant policies
• Conduct personalised conversations to understand individual needs, preferences, and goals.
Assess people’s needs and facilitate joint working across multidisciplinary teams.
Ensure effective flow of information between health, care, and social support services.
Monitor individuals’ needs and respond to changes promptly.
Manage a caseload of individuals, determined locally according to complexity and support requirements.
Support individuals in making informed decisions about their care and accessing appropriate services.
Follow safeguarding procedures and raise concerns appropriately.
Contribute to reducing the likelihood of acute or crisis care by proactive care management.
Collaborate with colleagues to improve service delivery and support neighbourhood, place, and system priorities. Skills and Competencies:
Strong interpersonal and communication skills, particularly in personalised conversations.
Ability to assess, prioritise, and manage complex care needs.
Knowledge of local health and care systems and services.
Competency in facilitating collaboration across teams.
Flexibility to adapt skills to specialist roles or a diverse range of conditions.
Commitment to safeguarding and ethical care practices.
Qualifications
• Proven experience in an office environment or administrative role is highly desirable
• Excellent organisational skills with the ability to manage multiple priorities efficiently
• Strong communication skills, both written and verbal, with a compassionate approach towards clients
• Ability to work independently as well as part of a team in a fast-paced setting
• Proficiency in office software applications such as MS Office (Word, Excel, Outlook)
• A genuine interest in healthcare or social services is advantageous but not essential

• Additional Information:
Care Co-ordinators require support from their employer to succeed in their role. This includes access to supervision, guidance on safeguarding procedures, and clarity on the purpose of the role within local service priorities.
Minimum of three years’ experience in a health or social care role is required.
