Crynodeb
To provide professional supervision to Occupational Therapy Assistants and students.
1.1 To provide a holistic initial assessment of service users with complex care and therapy needs
in the Reablement team
1.2 To support service users to set client centred goals to promote independence and wellbeing
in the home environment
1.3 To assess and progress mobility and balance and use standard physiotherapy outcome
measures to measure efficacy of treatment programmes
1.4 Assessment and provision of equipment to improve independence with activities of daily living.
1.5 To provide comprehensive falls assessment
1.6 To assess transfers to reduce risk of injury to service users and carers and where feasible
reduce packages of care for single handed care.
1.7 To support the duty manager to triage referrals in to the service when required, in order to
ensure that the appropriate pathways into Adult Social Care are identified.
1.8 To provide urgent home assessments when required, in order to facilitate timely discharges
from hospital or following functional deterioration of service users in the community.
