GP Intermediate Care/Telemedicine - 4PAs a week
We are seeking to recruit General Practitioners to join our team within the Intermediate Tier Services (Community Division) that provides care and treatment across our Intermediate Care and Virtual Ward services. All roles are part-time with opportunities for extra shifts. (4.5PA per week including SPA). We are looking to develop a team of GPs with interests in the care of older people and embed effective clinical leadership within our division. The roles are crucial to assist in driving forward the care and treatment within our intermediate care unit and hospital at home services with a clear focus on person-centered holistic care and treatment.
Stamford Unit is the intermediate tier bed base delivering the following pathways - rehabilitation, end of life care, step up from the community and complex discharge planning. As the accountable clinician, you will lead care and ward rounds for a cohort of patients. You are supported by a strong and effective team of Advanced Clinical Practitioners, Social workers, Therapists and Nursing teams.
As a connected team, our Hospital at Home services include support from Urgent Crisis Response teams, advanced nurse and paramedic assessors and extensive care services for respiratory and heart failure care. The role includes medical assessments using telemedicine services and care coordination.
GPs working a 1:4 weekend rota with cover for Stamford Unit and Virtual ward
MAIN RESPONSIBILITIES OF THE ROLE:Working closely with the Divisional Director, Directorate Manager, Divisional Medical Director, Divisional Nurse Director and Matrons in ensuring that a safe and effective patient service is delivered across all days of the week and bank holidays, in line with national and locally agreed standards ensuring effective and efficient use of resources.
Alongside the senior management team within the Directorate and Division facilitate delivery of objectives in partnership with colleagues.
To provide and share best practice.
To further a culture of continuous improvement across the Intermediate Care and Telemedicine services.
To contribute to the clinical governance agenda within the service and ensure appropriate governance arrangements are in place.
To contribute to audits, incidents and complaints; ensuring learning is embedded in the service
Work flexibly to support patients, colleagues and the wider service.
To contribute to clinical networks both internal and external to the Trust.
To ensure that the Division manages its income and expenditure budget within target and ensures optimum financial performance including identification and achievement of cost saving programmes
To operate within the required behaviours and values of the Trust. Lead by example in the delivery of the team’s performance.
