Job DescriptionJob Title Clinical Pharmacist Employer Winchester Rural South PCN Hours Full Time 37.5 hours per week Contract Employed or Self-Employed Salary Dependent on experience Job Purpose The Clinical Pharmacist will work as part of the multidisciplinary team within Winchester Rural South PCN to deliver high-quality, patient-facing and non-patient-facing clinical pharmacy services in line with the NHS 2025/26 Network Contract DES. The post holder will support medicines optimisation, improve patient outcomes, reduce medicines-related harm, and support safe, effective, and cost-efficient prescribing across the PCN. Key Responsibilities Deliver Structured Medication Reviews (SMRs) for patients with complex polypharmacy, frailty, long-term conditions, and high-risk medicines Provide clinical medication advice and support to GPs, nurses, and the wider multidisciplinary team Support medicines optimisation, including deprescribing, switching, and improving cost-effective prescribing Manage and resolve medication-related queries, including medicines reconciliation following hospital discharge Support repeat prescribing processes in line with local and national guidance Identify patients at risk of medicines-related harm and implement appropriate interventions Support long-term condition management, including cardiovascular disease, hypertension, diabetes, and respiratory conditions Undertake care home medication reviews where required Contribute to clinical audit, quality improvement projects, QOF, IIF, and PCN priorities Work collaboratively across multiple GP practices within the PCN Support prescribing governance, medicines safety alerts, and policy implementation Maintain accurate and timely clinical records using GP clinical systems Participate in ongoing training, appraisal, and professional development Professional Responsibilities Maintain GPhC registration and work within professional standards and scope of practice Work in accordance with PCN policies, NHS guidance, and information governance requirements Promote equality, diversity, and inclusion in line with NHS values Contribute positively to a supportive, learning-focused PCN environment Accountability The post holder is accountable to the PCN Clinical Director / designated GP supervisor and will work closely with practice teams and other ARRS roles.
