The Integrated Community Response Service is a large multidisciplinary team made up of GPs, occupational therapists, a social worker, physiotherapists, nurses, a community matron and health and social care assistants. The main aim of the service is to manage complex clinical cases at home, prevent unnecessary hospital admission and support early discharge of patients following an acute admission. This award winning rapid response team provides immediate assessment, intervention, early rehabilitation and management for a broad range of patients with complex needs. All members of the team work very closely together and have a good relationship with other community teams and clinicians such as district nurses, GPs, London Ambulance Service and mental health services.



We also have a number of forward-thinking quality initiatives developing within the team, e.g. Hospital at Home service working closely with West Middlesex Hospital and a lifting service throughout the borough working with LINKLINE and London Ambulance Service.



We are currently seeking a highly skilled and motivated individual to manage a complex caseload of patients and provide leadership for the occupational therapy element of the team. Alongside these duties, as part of the senior team, you will be expected to support the junior members of the multidisciplinary team and support the clinical services manager in day to day management and service development. You will have advanced risk assessment and management skills and preferably have previous experience with treating acutely unwell clients in community settings. You must be flexible, organised and have excellent time management skills in order to manage a case load effectively as the team priorities change frequently throughout the day.



You will need to be able to travel independently throughout the Borough of Hounslow to visit Patients. You will be required to work long days and weekends on a rota basis. The rota is created for a 4 week period.

Daily hours are 8am to 8pm. You will complete an average of 37.5 hours per week and complete 150 hours every 4 weeks.
