Crynodeb
To promote and deliver a high standard of care based on best practice to maintain patient/service user safety and provide an excellent. Act as the point of contact for patients experiencing an acute episode and liaise with colleagues to manage the condition appropriately. This may involve liaising with the specialist IBD nursing team in Southampton To coordinate the assessment and treatment of patients/service users with inflammatory bowel conditions. To coordinate the development, implementation and evaluation of specialised programmes of care utilising evidence and research. To provide specialist advice in acute and chronic situations ensuring that the patients/services users needs are met along with their support network. To ensure patient/service user involvement in the planning, delivery and evaluation of care, respecting their beliefs and dignity. To utilise data and information from a range of sources to identify patient/service user condition to guide decision making. Within field of expertise and policy and where appropriate, request specific clinical tests to monitor patients conditions, liaising with the consultant as required. To continue recording IBD patient data on a spreadsheet to provide an overview of patients receiving biological medication. This will support discussions when considering stopping medication and/or switching to subcutaneous or oral forms of treatment to help ensure best and most cost-effective use of public resources. To undertake the role of health promoter and educator for patients/service users and their support networks. To uphold own professional standards and work within the postholders sphere of responsibility. To manage all incidents as per policy and participate in risk management and Quality and Safety processes e.g. SWARM. To be accountable for ensuring that safeguards are in place for patients/service users and escalate any safety, safeguarding or risk concerns following the appropriate processes.
