Care Coordinator – Kind Tailored Care
Location: Beachley near Chepstow | Hours: Monday to Friday 9am-5pm | Salary: £25,000 – £26,000 DOE

We are looking for a values-driven Care Coordinator to ensure our service users receive reliable, high-quality care from familiar, well-matched staff, while supporting our care team to work safely, fairly, and effectively.

What you’ll do
As a Care Coordinator, you will:
Care Coordination & Rota Management
• Develop and maintain planned rolling rotas (3–6 months), ensuring all service user calls are covered by appropriately trained staff.
• Promote continuity of care by minimising unnecessary rota changes and supporting consistent staffing.
• Respond promptly to staff absences, ensuring all care calls are safely covered.
• Monitor and calculate service hours to align commissioned care hours with staff availability.
• Approve and report weekly staff hours accurately to support payroll processing.
• Manage annual leave in line with service needs, preventing understaffing and team pressure.
• Collaborate with Kind Recruitment Services to secure additional staffing when required.
Communication, Teamwork & Support
• Be a supportive point of contact for care staff, providing clear guidance for each shift.
• Maintain a professional and courteous telephone manner, reflecting the values of Kind Tailored Care.
• Ensure the right staff with the right skills are matched to each service.
• Handle on-call responsibilities during office hours and participate in the out-of-hours on-call rota.
• Escalate risks, concerns, or patterns affecting care quality to the Registered Manager promptly.
Systems, Accuracy & Continuous Improvement
• Use scheduling systems effectively and responsibly, reporting trends, errors, or system issues.
• Follow best practices in rota planning and system use, supporting consistency across services.
• Maintain accurate staff records, assign payroll codes correctly, and liaise with HR while maintaining confidentiality.
• Provide accurate weekly hours reports to finance, ensuring staffing remains safe and cost-effective.
• Take a solution-focused, accountable approach to problem-solving, always considering the impact on service users and staff.
