Crynodeb
The Band 5 Registered Nurse will work within Ward 14 Discharge Unit, a dedicated area supporting patients who are medically fit for discharge but require short-term care or coordination before leaving hospital. This role is essential in improving patient flow, reducing delays, and ensuring safe, timely discharge while maintaining high standards of infection prevention and patient care. Deliver high-quality, compassionate nursing care to patients awaiting discharge.Assess and monitor patients to confirm clinical readiness for discharge.Coordinate discharge plans, including medications, follow-up appointments, and community support. Work collaboratively with multidisciplinary teams to facilitate smooth patient transitions. Uphold infection prevention and control standards at all times. Support patients with complex needs during their hospital stay. Contribute to reducing hospital-acquired infections through best practice care.
