Crynodeb
The post holder will hold a certificate of Qualification in Social Work/ Diploma in Social Work/ DIPSW or Social Work Degree and fully registered under Social Work England Registration Body.
The role of a Social Worker involves working with people (including those with physical disabilities , learning disabilities and mental health needs), and their carers needs within their own environments to provide support with their care needs, and to help them attain an agreed level of independence
You will be working with care agency staff, occupational therapist, and physiotherapist and social workers. You will be required to develop and monitor reablement programmes to empower people with their daily living activities.
You will be required to undertake Safeguarding and Sec 42 inquiries.
As part of a multi-disciplinary team you will ensure client and carers’ wishes are central to the decision making and together formulate and implement appropriate care plans which will maximise their independence and help them to remain living in the community for as long as possible or assist them to move seamlessly into more appropriate accommodation.
