Summary


We are seeking an enthusiastic Physiotherapist or Occupational Therapist to join the Community Independence Service (CIS) and support older adults in Hammersmith & Fulham to live independently.

You will join a committed, multidisciplinary team delivering high-quality patient care across the Home First/Discharge to Assess pathway. Working alongside therapists, healthcare assistants, coordinators, Rapid Response, Reablement and Social Care colleagues, you will support safe and timely discharges into the community. The service is co-located with LBHF Adult Social Care in Hammersmith and works closely with Charing Cross Hospital and other local acute partners. You will also be connected with peers across Hammersmith & Fulham, Westminster, and Kensington & Chelsea, contributing to service development and partnership working.

As part of the CIS Home First team, you will be a flexible and skilled clinician with a broad range of competencies. Using evidence-based practice, you will assess, plan, implement and evaluate interventions, delivering physiotherapy or occupational therapy care in patients’ homes following holistic and risk-based assessments and agreed care plans.

This role offers the opportunity to make a meaningful difference through face-to-face care within a supportive multi-agency team. We actively support mobile and flexible working.

The service is operational7 days a week 8 am to 8 pm. Hours of work for therapists will vary between these hours.





You will have excellent communication skills, IT skills, and the ability to be adaptable, flexible and reliable as changes to your caseload and patient visits may happen on a daily basis. You will therefore require good organisation skills and the ability to prioritise your workload effectively. Experience of care in a community setting would be an advantage but is not essential.

1. Carry out day to day care of patients within their own home environment, such as personal care, medication prompting, toileting and meal preparation.
1. Carry out safe moving and handling of patients with the use of equipment prescribed by Occupational Therapists and Physiotherapists, for example rollator frame, sara steady, re-turn and hoist.
2. Review and adjust basic equipment that has been prescribed and installed by Occupational Therapists or Physiotherapists, for example mobility aids or toilet frames.
3. To be able to take observations (for example blood pressure and temperature) after completing the appropriate competency, NEWS2 scoring and early identification and escalation of the deteriorating patient.
4. Participate and contribute to audits, service development and projects associated with improving the quality of service of CIS.
