Crynodeb
Main duties include, Comprehensive assessment, diagnosis, management and follow-up of patients presenting with acute and chronic conditions Prescribing or managing medication for patients Independent Prescriber status required Working autonomously within agreed clinical governance frameworks, with access to clinical supervision as needed Supporting the wider practice team: liaising with GPs, nurses, other clinical staff, admin ensuring continuity of care Maintaining accurate patient records in line with NHS / CQC standards Participating in quality improvement, audit, referral decisions, and clinical governance
