OUR VALUES Collaborative Responsive Thoughtful Courageous JOB SUMMARY We are seeking a dynamic and compassionate Rehabilitation Assistant to join the Rehabilitation Team, providing specialist care for palliative patients. The main focus of this role is to help patients achieve their goals and maximise their mobility and function. This is an exciting opportunity to make a real difference in improving patients' quality of life through expert assessment, treatment, and education. You will work both autonomously and as part of a supportive team to deliver high-quality, person-centred care for in-patients and outpatients in our growing Wellbeing service. You will lead a Wellbeing exercise programme and guide patients through their personalised exercises plan. Key Responsibilities Provide clinical and administrative support to the Rehabilitation Team in delivering palliative rehabilitation. Carry out moving and handling assessments, including prescribing and ongoing evaluation of equipment. Maintain accurate patient record in accordance with policies. Empower patients through self-management strategies and education. Collaborate with equipment suppliers, GPs, community nurses, physiotherapists, and other healthcare professionals to arrange smooth and safe discharges when needed, and order appropriate equipment. Support service development and participate in group sessions and staff training Deliver breathlessness, fatigue, and anxiety management classes and sessions. Assessing and provide mobility aids; ensure patients receive relevant information and support.
