To work with registered professional within the remit of the individuals care plan To document relevant information relating to own activity in accordance with Trust policies and procedures, contributing to updating patient records, under supervision of the registered professional, using the appropriate manual or electronic system. To participate as a member of the Multi-Disciplinary team. To promote positive relationships with service users and carers Foster and promote relationships with community support mechanisms, Voluntary agencies to enable social inclusion To ensure that all care is provided in line with Trust values. To ensure that all interactions with service receivers are used as opportunities to interact effectively to provide a high quality caring experience, and where possible, opportunities for positive change. To promote effective communication which may include the need to encourage the service receiver and involve the use of tact. It may be necessary to adapt communication techniques in order to facilitate a service user understanding and engagement.
