We are a team of RN, Advanced Clinical Practitioners, Health care Assistants, Physiotherapist, Occupational therapist, Speech and Language Therapist, Rehabilitation Support Workers, Social Services team and a Ward Doctor who aim to maximize our patients potential to enable them to return home. We have a visiting Stroke and a Geriatric Consultant both from an acute hospital who see to and review stroke and medical patients respectively once a week. When that is not possible, we work closely with our Social care colleagues to plan the best outcome.

A health care assistant who applies the importance of prioritization and supports the team assertively. It is hard work but very rewarding-planning the patient pathway, enabling them to progress and supporting them through setbacks is what we do best.

As this is a 24 hours service, internal rotation will be operating.

If you would like to pay us a visit to see what we do and how we do, please call our Ward Manager Cherrie Lynn Leeping Pineda on 01273 696011 ext 6053.
