Primary Duties and Areas of Responsibility Supports health promotion and prevention activities across the PCN, including smoking cessation, weight management, and lifestyle interventions. Play a key role in vaccination and immunisation programmes within the PCN. Work within a Multi-Disciplinary Team to support patients with long-term conditions and health inequalities. Provides clinical support to registered nurses and other healthcare professionals within the PCN. Delivers basic clinical care interventions under supervision, supporting continuity of care and patient safety initiatives. Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, recording findings accurately in the patients health care record using our Primary Care software system, SystmOne. Run chronic disease clinics such as Hypertension Reviews and Diabetic Foot Checks. Carry out BMI checks as directed. Act as a chaperone as required. Undertake venepuncture. Carry out 24 hour ECG as requested. Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the practice IPC policy. Deliver opportunistic health promotion where appropriate. Ensure when significant events are identified, they are recorded appropriately on GP Team Net. Skill & Attributes: Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Strong organisational and time-management abilities. Commitment to patient-centred care and clinical excellence. IT literacy and familiarity with clinical systems. Must have the ability to travel locally for home visits, which includes having access to own vehicle with the relevant business insurance.
