Summary


e are seeking an experienced individual to support the next stages in the development of the newly established CAMHS transitions service. This role will be a pilot, and it will run for 12 months initially, but the hope is that this role will become permanent, so we can continue to support young people from our CAMHS inpatient settings to the next stage in their recovery.

As a team we adopt a holistic approach, not only focusing on mental health but also addressing the physical, social, educational, and motivational needs of the young people we work with. By employing the "social prescribing" model, we help young people engage with life through activities that promote community connections, sports, education, employment, housing, and more. Your role will empower young people to build confidence, improve wellbeing, and work towards their long-term health goals.

We work in both the inpatient and community settings. We work from Kent and Medway Adolescent Hospital. We work with multiple services, and we will often have daily communications with community CAMHS, Children’s Services, education providers, third-party services, adult services.





The purpose of this role is:



· To liaise with relevant agencies in the delivery of care to young people and their families.



· To provide time limited evidence based therapeutic interventions and to provide support to young people and their families/carers following discharge from inpatient care.



· To promote and facilitate the participation of service users and carers in their own care and in the planning and development of services.



· To take clinical lead and full line management responsibility for transition service ensuring they deliver a high quality, young person focused service.



· To lead and manage referrals into the Transition Service.



· To lead on and undertake outreach and group work to support young people and families.



· To work with colleagues within Child and Adolescent Mental Health Services to ensure delivery of comprehensive after care from inpatient admission.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.


Probationary Period

This post will be subject to a probationary period.
