Crynodeb
We are looking for a salaried GP to cover 6 sessions, Monday - Wednesday. Duties will include: Surgery consultations, triaging calls as appropriate, occasional home visiting & administration associated with patient care & to run duty sessions. Work as a team member with all relevant providers of primary and community care services within the Practice. Ensure appropriate records are maintained at all times regarding patient care & that relevant team members are kept fully informed regarding patients seen on behalf of the Practice. Ensure that the Practice achieves the highest QOF outcomes. Developing care plans for health in consultation with patients & in line with current practice disease management protocols. Support the proactive care agenda by participating in care planning & multi-disciplinary meetings. Attend team & healthcare meetings when required & represent NMC Partnership at external meetings. We support candidates if they wish to be looking to further their skills or become a trainer or do Gypsi roles.
