Accountable for own actions in accordance with NMC Code of Professional Conduct. Will contribute towards corporate objectives, acting within local, Trust and statutory guidelines and policies at all times. Responsible for undertaking holistic specialist palliative care assessments, identifying and diagnosing patients physical, social, psychological, cultural and spiritual needs to facilitate the provision of quality nursing care; contribute to decisions regarding clinical care andpriorities. Attend and initiate case conferences as appropriate To attend weekly Specialist Palliative Care Multidisciplinary Team meetings to discuss patient caseloads. Undertake highly specialised technical and/or practical skills training to ensure optimum patient management including; complex patient monitoring; equipment management and defined expanded clinical roles e.g. non-medical prescribing. Regularly manage expected and unexpected clinical events requiring high but unpredictable levels of physical effort according to patient dependency/clinical need. Support patients, carers and staff during difficult situations arising in the clinical area, e.g. breaking bad news or following an unexpected event. Provide help throughout the hospital to clinical teams in identification of patients with palliative care needs through increased visibility and presence on ward or board rounds in care of the elderly / frailty services, cardiac failure services, respiratory services and others.This will lead to earlier identification of patients who need advanced care planning, and reduction in length of stay and investigation of these patients. This would in turn also help to reduce the chance of future ED attendances for these patients. Be a visible presence in ED and AMU. Identification of patients in the Emergency Department who may benefit from palliative care interventions that enable them to be discharged with an early plan to be seen in an outpatient setting or in the community (through close liaison with community services).
