Summary
We are seeking to recruit an Urgent Response Support Worker.
The successful candidate will be a key member of a multi-disciplinary team providing support to adults who are returning to their own home after a stay in hospital or from reablement unit. These individuals require support to regain their independence during times when there is an alteration in their physical or psychological well-being.
This role provides a high standard of support within a reabling ‘ethos’ which promotes independence and improved quality of life, dignity and choice undertaking medical, nursing or therapy related treatment programmes or social care intervention in order to support and enable people to return to independent living.
We are looking for enthusiastic, motivated, forward-thinking individuals who can actively encourage and enable people to live independently and remain an active part of their community.
Applications should demonstrate:
• Previous experience in working with people delivering care or support
• Hold a UK Driving Licence and have access to a car
• A qualification in health and social care would be an advantage
The role requires a flexible approach to work rotas as the service operates a 5 day out of 7 rolling rota, including weekends and Bank Holidays, between 7am-10pm.
