Key Tasks Management and reconciliation of medicines at discharge from Hospital, intermediate care, care homes; Communicating with changes to patients medications with community pharmacists; Working as part of the multi-disciplinary team to lead medicines management across the practice; Take responsibility for prescribing aspects of the ICB Practice Engagement Scheme and other incentive schemes Supervision of foundation pharmacists. Responsibilities underpinning the role Undertake high standard structured medication reviews with patients and produce recommendations for nurses and/or GP on prescribing and monitoring. Set up and manage systems to ensure continuity of medicines supply to highrisk groups of patients e.g. those with medicine compliance aids or those in care homes. To develop and facilitate a good working relationship with community pharmacists and other local providers of healthcare; To plan and organise the post holders own workload, including audit and project work, and training sessions for members of the practice team; To record personally generated information and maintain a database of information relating to the work done in the practice personally generated information includes information and records relating to audit and clinical work undertaken by the post holder, reference notes relating to clinical/technical information, etc the database includes maintaining up-to-date, detailed records of all work done in the practices for which the post holder is accountable; To maintain registration as a pharmacist and comply with appropriate professional codes; As appropriate to the post, to maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments, agree objectives and a personal development plan and participate in the appraisal process; Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved; Contribute to ensuring that practice medicines management policies are kept up to date and adhered to; Where applicable, contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets); Working with the Link Workers and Care Coordinators to support public health campaigns and to provide specialist knowledge on all public health programmes available to the general public; Provide education and training to primary healthcare team on therapeutics and medicines optimisation; Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.
