Patient Assessment & Care Conduct comprehensive clinical assessments of patients presenting with acute illness or injury. Deliver appropriate treatment, management plans, and clinical interventions within your scope of practice. Prioritise patient needs based on clinical urgency and safety. Coordination & Referral Collaborate with GPs, paramedics, and other healthcare professionals to ensure continuity and smooth transition of care. Refer patients to relevant services, specialists, or emergency departments in line with clinical pathways. Patient Education & Support Provide clear, tailored information to patients and families about condition management, medication use, and safety-netting advice. Offer guidance on lifestyle improvements and when to seek further medical attention. Documentation & Record-Keeping Maintain accurate, comprehensive, and timely clinical documentation in accordance with legal, professional, and organisational standards. Record all patient interactions using appropriate systems and coding protocols. Professional Development Engage in continuous professional development, reflective practice, and relevant training to stay up to date with clinical best practice and protocols.
