Are you an experienced senior nurse with strong leadership skills and a passion for delivering exceptional, relationship-centred care? We’re looking for a Community Lead Nurse to join our team and play a key role in ensuring our residents receive the highest standard of support every single day.
In this role, you’ll lead your allocated community/unit, inspiring and guiding your team while ensuring care is delivered in line with Hallmark’s values, vision and policies.
What You’ll Do
Lead high-quality, relationship-centred clinical care within your community.
Act as Duty Manager when required.
Support recruitment, training, development and induction of team members.
Manage rotas to maintain safe staffing and the right skill mix.
Motivate, guide and performance‑manage the team to deliver excellent outcomes.
Communicate effectively with residents, families, colleagues and external partners.
Ensure alignment with Hallmark’s values, best practice and policies.
Lead and audit care planning, pre-admission assessments and resident reviews.
Oversee safe and compliant medication management.
Contribute to practice development, service improvement and audit actions.
Maintain Health & Safety standards: complete and review risk assessments; report concerns, unsafe practices, incidents and near misses.
Skills & Experience Needed
NMC registration and experience as a senior nurse in a care home setting.
NVQ Level 3 in Health & Social Care.
Registered with Social Care Wales (where applicable).
Willing to work towards ILM Level 5.
Strong communication and leadership skills.
Approachable, respectful, confidential and transparent.
Adaptable, solutions-focused, and committed to high-quality care.
Confident using electronic care planning, medication and compliance systems.
