The role will involve working with individuals with a complex psychosis from assessment of rehabilitation needs, through care planning interventions to build on skills and increase independence and developing safety and risk management plans for individuals in the community. You will coordinate the care of a caseload of individuals, whilst working alongside a diverse MDT supporting delivery of a range of interventions to support the rehabilitation of this population both across the community, but also those who require inpatient rehabilitation, or an episode of acute care. In addition to direct patient care there will also be coordination of the shift through a duty worker role ensuring safe and effective care, management of lone working and prioritisation of clinic care across the team through a duty worker system. The team will work closely with colleagues from other teams in the pathway, ensuring service users are accessing the support that meets their needs. As a senior rehabilitation practitioner, you will also have a role in supporting service development through lead roles, and supervision of others. Travel will be required across the West Yorkshire Area. There will also be some travel to out of area hospitals outside of the West Yorkshire Footprint as part of assessment and engagement, in addition to supporting service users funded by Leeds but supported in placements within west Yorkshire region.
