Crynodeb
Provides expert clinical leadership in dementia and delirium care, acting as an autonomous practitioner accountable to professional, Trust and statutory standards. Leads service development, quality improvement, education and implementation of the organisations dementia strategy, while delivering specialist assessment, care planning and clinical guidance for patients, families and staff. Works collaboratively across multidisciplinary and external services, ensuring effective communication, governance, evidence-based practice, audit and research. Promotes high-quality, person-centred care, supports staff development, and drives continuous improvement informed by policy, data, and patient and carer experience.
