Clinical 1. Participate in the PROPEL programme (UHP rotational development programme), undertaking defined and structured Band 6 core or Band 7 specialist rotations across a variety of different clinical specialities, as defined above. 2. To participate in all activities of the pharmacy dispensary as required e.g. clinical screening and final accuracy checking of prescriptions. 3. To participate in ward based clinical pharmacy services to support the safe prescribing and administrations of medicines. This may include activities drug history taking and medicines reconciliation, medicine supply, patient medication consultations, medicines optimisation services, discharge medication service (DMS) and any other activities associated with medicines management. 4. Perform clinical check of prescription charts for individual patients and ensure safety promoting medicines optimisation communicating any problems and negotiating solutions with doctors and/or ward staff, in the ward or dispensary. 5. To participate in the production and supply of aseptically dispensed items, whilst working in the aseptic unit. 6. To provide medication related advice to patients, focussing on medicines stopped/started or changed in hospital and side effects profiles enabling them to obtain the maximum benefit from their medicines. 7. Ensure all relevant documentation is completed and document any actions appropriately in the patients notes and / or electronic patient record. 8. To facilitate timely medication availability through participation in, and further development of, the existing patients-own drugs and onestop dispensing schemes. 9. Provide professional responses to medicine information queries from within a clinical service line or designated care area.
