Crynodeb
The care-coordinator position is an integral part of the Mental Health Inreach Team, which delivers high-quality, evidence-based mental health care within the prison environment. Treatment is shaped through needs-driven assessments, personalised care planning, and comprehensive risk management tailored to the prison population. The team operates through a genuinely collaborative, multidisciplinary, and multiagency model, aiming to reduce distress while promoting autonomy, partnership, and recovery. The postholder will work closely with colleagues to support the Trusts commitment to providing consistent care and support for individuals with mental health needs in custody. The role demands the ability to respond effectively to psychiatric emergencies and crises across the prison, with an expectation to fulfil the responsibilities of the duty worker role.
