Job Purpose The Nursing Associate will work as part of the Primary Care Network multidisciplinary team, supporting the delivery of high-quality, patient-centred care across five GP practices. The role bridges the gap between Health Care Assistants and Registered Nurses, contributing to clinical care, health promotion, long-term condition management, and prevention in line with PCN and ARRS objectives. The post holder will work flexibly across practices to support workforce resilience, continuity of care, and improved patient outcomes. Key Responsibilities Clinical Care Deliver safe, effective, and evidence-based nursing care under the direction of a Registered Nurse or GP Undertake delegated clinical tasks, including: Phlebotomy and venepuncture Blood pressure monitoring ECG recording Wound care and dressings Injections and immunisations (where trained and competent) Health checks and screening procedures Support long-term condition reviews (e.g. diabetes, asthma, COPD, hypertension) Recognise and escalate clinical concerns appropriately Patient-Centred Care Deliver care in line with individual care plans and agreed protocols Support health promotion and self-management Communicate effectively with patients, carers, and families Promote equality, diversity, and inclusion in all patient interactions PCN & Practice Working Work across five GP practices, adapting to different teams and environments Participate in PCN multidisciplinary team meetings Support PCN priorities such as: Preventative care and early intervention Long-term condition management Reducing health inequalities Vaccination and immunisation programmes Documentation & Information Governance Maintain accurate, timely, and contemporaneous clinical records using SystmOne Ensure compliance with information governance, GDPR, and confidentiality standards Contribute to data collection and reporting for ARRS and PCN DES requirements Professional Practice & Development Practice in accordance with the Nursing and Midwifery Council (NMC) Code Maintain Nursing Associate registration and revalidation requirements Work within own scope of practice and competence Engage in clinical supervision, appraisal, and continuous professional development Contribute to service improvement and quality initiatives Team Working & Communication Work collaboratively within the PCN multidisciplinary team Support and mentor healthcare assistants where appropriate Communicate effectively with GPs, nurses, allied health professionals, and administrative staff Person Specification Essential Foundation Degree or equivalent qualification as a Nursing Associate Registered with the Nursing and Midwifery Council (NMC) Experience working in a health or care setting Ability to work across multiple GP practices Strong communication and interpersonal skills Ability to work within scope of practice and follow clinical protocols Competent IT skills and accurate clinical record keeping Desirable Experience working in primary care or GP practice settings Experience using SystmOne Experience supporting long-term condition management Evidence of ongoing professional development Values & Behaviours Compassionate and patient-focused Professional and accountable Flexible and adaptable Team-orientated and collaborative
