The post holder will play a key role as part of the Continuing Healthcare (CHC) and complex care team alongside the CHC, Finance and Performance Teams. They will ensure that the CHC system(s) are used to capture and record patient information is kept up to date and functionality understood and supported across the whole CHC team. This will include supporting the management, reporting and monitoring of data contained within the CHC system for performance management, in line with national requirements. The post holder will help ensure data generated by the Complex Care Team is captured within the CHC system so that the system is comprehensive and robust and allows for the accurate management of caseloads and the presentation of that data. The interface with finance allows for the most up to date data and information to be used for operational and strategic forecasting and modelling whilst supporting the day-to-day clinical team with operational decisions and planning. The post holder will check the validity of the data, before sharing with: CHC and Complex Care Team Management Group; Sign off regarding accuracy will be required prior to submissions to Regional and National Bodies who have a need to receive that data. The postholder will need to maintain strong links with the ICB Analytics / Business Intelligence team as part of the monthly and quarterly performance reporting requirements. The post holder will work as a superuser for CHC systems, providing support to members of the Complex Care team. The post holder will provide complex reports produced using data from CHC at the request of the Complex Care Management Team. The post holder will play a key role in supporting the Complex Care area of the ICB. Working within the remit of the National Framework for Continuing Healthcare, information governance and performance management the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service. The post holder will be required to work with minimal supervision and using their own initiative as well as working as part of the wider complex care ICB team. They will demonstrate a willingness to help others and be a key member of the team with responsibility for specifically allocated tasks. The post holder will be expected to follow set processes, prioritise their own workload and demonstrate initiative in order to fulfil the requirements of the post. Excellent interpersonal skills and communication skills are essential plus an ability to deal with and manage highly sensitive and confidential information. The post holder must have a flexible and adaptable approach to their work in order to meet demanding deadlines and have the ability and resilience to regularly deal with a range of routine and non-routine administrative and data tasks as required to deliver an efficient and effective service. The role will be responsible for supporting the ICB levelling up CHC systems to ensure functionality is consistent across operational, finance and performance and support with the potential mergers of systems.
