Crynodeb
Responsible for developing, with patients, a continuing management plan for the health of the patient and when appropriate to refer to other clinicians and agencies The Paramedic will support the network, required to attend same day visits booked in by the surgeries/care homes to support patients in the community. The successful candidate will work within the multidisciplinary team to deliver the requirements of the QOF, DES and IIF framework as part of the primary care network. The role will involve independent prescribing within the scope of professional practice.
