To contribute as a key member of the CAMHS Transitions Team, providing high-quality care to young people and developing, planning, and implementing individualised treatment programmes based on the specific needs of each young person. To critically assess and evaluate the effectiveness of care programmes, working collaboratively with the multidisciplinary team to ensure optimal outcomes. To provide expert clinical knowledge and skills across both inpatient and community settings, delivering evidence-based interventions and direct care. To manage caseloads autonomously, ensuring effective time management while delivering care and support to young people in inpatients and community. To monitor and assess the impact of prescribed medication, providing timely updates to the line manager and ensuring the wider team is informed of any changes, fostering effective communication across teams. To engage young people in a person-centred approach to goal setting and treatment planning, ensuring that their views and those of significant others are integral to the formulation of care plans. To actively contribute to the development and improvement of the transition service, supporting the implementation of service changes and contributing to the development of new initiatives within the team. To participate in professional meetings, including multi agency risk planning meeting reviews, contributing to multi-disciplinary discussions to ensure high-quality care is provided. To maintain accurate and up-to-date records of care programmes and communications, ensuring compliance with the electronic patient record system (Rio). To demonstrate sound knowledge of and adhere to the principles of the Mental Health Act 1983 in practice. To collaborate with the wider professional network, contributing to care planning and ensuring integrated support for young people. To prioritise safeguarding in all aspects of clinical practice, attend relevant safeguarding training, address concerns with safeguarding leads, and ensure that safeguarding issues are appropriately recorded on the electronic patient record system Certificates of SponsorshipAlthough we are a registered sponsor organisation, we are unable to offer sponsorship for all job roles.
